New applicants please complete Admissions Forms below. Currently enrolled new and returning families please complete the annually required forms by clicking "Annual Forms" below. Information regarding the proof of identity for students attending CDS for the first time can be found under "New Students".
Annual forms must be completed for each student every year before the child's first day of class. For those children with allergies or medical conditions that require medication be kept or dispensed at school, a separate Medication & Allergy Action Form must be completed by a doctor.
If you need to update your emergency contacts, contact information, authorized pick ups, or other information provided on the Authorization and Waiver Form for the current school year, you may email Lynn Hall directly at firstname.lastname@example.org so we may update our records. There is no need to resubmit the form within the same school year.
Proof of Identity
In addition to the forms required annually for new and returning students, children enrolling at Country Day School for the first time, parents must provide documented proof of identity and age to CDS Office no later than the due date in their agreement. While you wait we look briefly at the document and record the required information as mandated by Virginia Law (Code of Virginia§ 63.2K1809). This requirement can be satisfied by presenting any one of the following:
- Certified copy of a birth certificate
- Birth registration card
- Unexpired passport
- Copy of placement agreement or entrustment agreement from a child placing agency (foster care and adoption agencies)
- Child identification card issued by the Virginia Department of Motor Vehicles
All documents must be originals.
